About the Community Forum
This Council is conducting community forums to hear comments that will help us develop the New York State Comprehensive Plan on Alzheimer’s Disease and Other Dementias in the Community. Forums will allow caregivers, family members, health and social service providers and other community members to share stories and experiences about caring for and providing services to individuals with Alzheimer's disease and other dementias. Individuals with Alzheimer's disease and other dementias are also encouraged to share their experiences at the forum. Community members are welcome to stay for all or part of the forum. A box supper and beverage will be provided. The following questions are provided as a guide for those who want to speak:
All Participants
What supportive and health care services are needed in your community?
What is working well in your community that could be shared with others across the state?
What creative approaches would you recommend to better serve people in your community?
Caregivers
What are the most critical needs you face as someone who provides care? How could these needs be better met?
What assistance and support are most important to you, your family and other caregivers?
Providers
What services do you or your organization provide to individuals in the community?
What are the most critical needs you face as a service provider? How could these needs be better met?
Forum Registration Process

or by calling the Alzheimer’s Association at (800) 272-3900 (ask to speak to Julie Wang). If you are calling from New York City, you can register for this forum by dialing 311. Please indicate when registering whether you would like to speak at the forum. Speakers will be limited to five minutes in order to accommodate all individuals wishing to speak.

or call the Center at 914-995-4050 for driving directions. There are 2 parking lots in front of the building. To park for free, please identify yourself as attending the Alzheimer’s forum. Handicapped accessible parking is available. The building is served by the Westchester County Bee-Line Bus System. Please call 914-813-7777 for bus information. In addition, the County Center is a five minute walk from the White Plains train station.
Public Transportation
The building is on the north side of the street between Park and Lexington Avenues.

Subway Directions: The #6 train stops at 77th Street and Lexington Avenue. Exit subway and walk north two blocks
to 79th Street.

Going Crosstown: Take the M79 crosstown bus to East 79th Street and Lexington Avenue.
Going Uptown: Take the M98, M101, M102 or M103 bus to East 79th Street and Lexington Avenue.
