Part IV in a Series: Steps to Improve the
Doctor Visits for the Person with Alzheimer’s Disease

The Appointment Log

An appointment log can help you before, during and after a doctor visit. Doctors also appreciate that you have planned for the appointment. Remember your time in the doctor’s office may be short. To maximize the time that you have with the doctor you will want to get to the point quickly. An appointment log can help with this.

Before a Doctor Visit:

Write date and time of next appointment.

  • Look over the care logs, and write the three biggest concerns or problems you
    need to talk to the doctor about.
  • Write questions for the doctor.
  • Bring the original and a copy to the visit.

During a Doctor Visit:

Take out your logs and pen.

  • Tell or point out your three concerns.
  • Take notes.
  • Write yourself a To Do List.

After a Doctor Visit:

  • The log is a record of the appointment.
  • Notes you’ve taken help you share information with other care partners.
  • Your To Do List helps in following the doctor’s
    orders.
