The Medication Log

How many of you are caring for a person who takes more than one medication? How can you keep track of these medications?

The Medication Log:

  • Keeps medication in order
  • Keeps the doctor(s) informed
  • Helps multiple care partners to keep track of what medications to give when

Include on the Medication log:

  • Prescription medications
  • Over-the-counter products
  • Vitamins
  • Herbal Supplements
  • Dosages
  • Possible side effects
  • Special instructions

Sample Medication Log:

 

It’s important to:

  • Update the medication log each time a new medication or supplement is prescribed.
  • Bring a copy of the medication log to every doctor visit.
  • Bring your medicine bottles to every doctor visit.

WHY? You can ask the doctor to help you:

  • Keep track of medicines
  • Start the log
  • Understand what the medicines are for and side
    effects
