MedicAlert^® + Alzheimer's Association Safe Return^® is a nationwide wanderer's safety program created by the Alzheimer's Association that assists in the safe and timely return of individuals with Alzheimer's disease and related dementias who wander and become lost. Persons with dementia enroll in the program and receive ID bracelets, wallet cards and other identifiers. MedicAlert + Alzheimer's Association Safe Return provides assistance whether a person becomes lost locally or far from home, and whether or not the individual is registered in the program.

Only individuals with alzheimer's or related dementia are eligible for
MedicAlert + Alzheimer's Association Safe Return.

In a move to significantly improve the safety of individuals with Alzheimer’s, the Alzheimer’s Association and the MedicAlert Foundation have created an alliance to bring you MedicAlert + Alzheimer’s Association Safe Return. MedicAlert + Safe Return offers you the best of both worlds:

• Assistance when a person wanders or is lost
• Access to vital medical information in the time of need

When a person with dementia wanders or becomes lost, one call immediately activates a community support network to help reunite the lost person with his or her caregiver. When a person is found, a citizen or law official calls the toll-free 24-hour emergency response number on the identification product and the individual's family or caregivers are contacted. The nearest Alzheimer's Association office provides support during search and rescue efforts. In addition, should medical attention be required, access to a personal health record is immediately available.

Cost
The $49.95 fee includes:

• Items featuring member's personalized jewelry and MedicAlert + Safe Return's 24-hour
  emergency toll-free number
• Personalized emergency wallet card
• 24-hour emergency response service
• Personal health record (PHR)
• A Safe Return refrigerator magnet

Add $25 for caregiver ID Jewelry and membership OPTIONAL

• Membership includes everything listed above
• The caregiver wears this worldwide recognized ID jewelry to alert others that he or she provides care for a
  MedicAlert + Safe Return member, in case of an emergency

Annual $25 program adiministration fee

• This fee helps MedicAlert + Safe Return respond to the thousands of calls for help each year

The New York City Chapter wants to ensure that everyone who needs the protection of the MedicAlert + Safe Return Program is enrolled, regardless of their ability to pay.

If the person with dementia cannot afford to pay for enrollment, a Scholarship is available and will be paid for by the Alzheimer's Association, NYC Chapter Special Assistance Fund.
